I update two tuples, so I manually CatalogOpenIndexes() and CatalogIndexInsert() two times, as per comments in CatalogUpdateIndexes(). I also removed a couple of useless CommandCounterIncrement(), some useless definitions in src/include/commands/cluster.h and useless includes in src/backend/commands/cluster.c.
