libpq: change PQconndefaults() to ignore invalid service files

Browse Source 
Previously missing or invalid service files returned NULL.  Also fix
pg_upgrade to report "out of memory" for a null return from
PQconndefaults().

Patch by Steve Singer, rewritten by me
