Note that this moves the public key check out of DH compute_key() since
key validation does not belong inside this primitive..
The check has been moved to the EVP_PKEY_derive_set_peer() function so that
it generally applies to all exchange operations.. Use EVP_PKEY_derive_set_peer_ex()
to disable this behaviour.

Replace "=for openssl foreign manuals" with simpler syntax, it looks
like the "=for openssl ifdef" construct.
Fix some broken L<> links; add some missing foreign references and fixed
some typo's.
The WARNINGS in dhparam referred to non-existant commands so reword it.

Because the algorithm to use is decided already when creating an
EVP_PKEY_CTX regardless of how it was created, it turns out that it's
unnecessary to provide the KEYEXCH method explicitly, and rather
always have it be fetched implicitly.
This means fewer changes for applications that want to use new key
exchange algorithms / implementations.

Previous commits added the EVP_KEYEXCH type for representing key exchange
algorithms. They also added various functions for fetching and using them,
so we document all of those functions.

While stereotyped repetitions are frowned upon in literature, they
serve a useful purpose in manual pages, because it is easier for
the user to find certain information if it is always presented in
the same way. For that reason, this commit harmonizes the varying
formulations in the HISTORY section about which functions, flags,
etc. were added in which OpenSSL version.
It also attempts to make the pod files more grep friendly by
avoiding to insert line breaks between the symbol names and the
corresponding version number in which they were introduced
(wherever possible). Some punctuation and typographical errors
were fixed on the way.
