FIPS module code *always* runs within the scope of an associated
OPENSSL_CTX. When the module is loaded the OPENSSL_CTX gets created, and
when the module is unloaded the OPENSSL_CX gets freed. A module may be
loaded multiple times within the scope of different OPENSSL_CTX objects.
"Global" data should always be stored within the OPENSSL_CTX. In this
way it will always get cleaned up properly when the module is unloaded.
All current code within the FIPS module works this way. To avoid
"accidents" we disabled the RUN_ONCE code inside the FIPS module.

Trim trailing whitespace. It doesn't match OpenSSL coding standards,
AFAICT, and it can cause problems with git tooling.
Trailing whitespace remains in test data and external source.

We have a number of instances where there are multiple "init" functions for
a single CRYPTO_ONCE variable, e.g. to load config automatically or to not
load config automatically. Unfortunately the RUN_ONCE mechanism was not
correctly giving the right return value where an alternative init function
was being used.

RUN_ONCE really just returns 0 on failure or whatever the init
function returned. By convention, however, the init function must
return 0 on failure and 1 on success. This needed to be clarified.

Because pthread_once() takes a function taking no argument and
returning nothing, and we want to be able to check if they're
successful, we define a few internal macros to get around the issue.
