FreeBSD also defines {make|swap|get|set}context for backward
compatibility, despite also exposing POSIX_VERSION 200809L
in FreeBSD 15-current.
Note: There's no fallback for POSIX_VERSION 200809 without
these routines, so maybe that should be a #error?

Since OPENSSL_malloc() and friends report ERR_R_MALLOC_FAILURE, and
at least handle the file name and line number they are called from,
there's no need to report ERR_R_MALLOC_FAILURE where they are called
directly, or when SSLfatal() and RLAYERfatal() is used, the reason
`ERR_R_MALLOC_FAILURE` is changed to `ERR_R_CRYPTO_LIB`.
There were a number of places where `ERR_R_MALLOC_FAILURE` was reported
even though it was a function from a different sub-system that was
called. Those places are changed to report ERR_R_{lib}_LIB, where
{lib} is the name of that sub-system.
Some of them are tricky to get right, as we have a lot of functions
that belong in the ASN1 sub-system, and all the `sk_` calls or from
the CRYPTO sub-system.
Some extra adaptation was necessary where there were custom OPENSSL_malloc()
wrappers, and some bugs are fixed alongside these changes.

Reverting to using swapcontext() when compiling with clang on BTI-enabled
builds fixes the BTI setjmp() failure seen when running asynctest.
The issue with setjmp/longjmp is a known clang bug: see
https://github.com/llvm/llvm-project/issues/48888

On IA64 the use of setjmp()/ longjmp() does not properly save the
state of the register stack engine (RSE) and requires extra care.
The use of it in the async interface led to a failure in the
test_async.t test since its introduction in 1.1.0 series.
Instead of properly adding the needed assembly bits here use the
swapcontext() function which properly saves the whole context.

CreateFiber and ConvertThreadToFiber are not allowed in Windows Store
(Universal Windows Platform) apps since they have been replaced by
their Ex variants which have a new dwFlags parameter.
This flag allows the fiber to do floating-point arithmetic in the
fiber on x86, which would silently cause corruption otherwise since
the floating-point state is not switched by default.
Switch to these "new" APIs which were added in Vista.

When Intel CET is enabled, makecontext will create a different shadow
stack for each context. async_fibre_swapcontext cannot use _longjmp.
It must call swapcontext to swap shadow stack as well as normal stack.

Make the include guards consistent by renaming them systematically according
to the naming conventions below
For the public header files (in the 'include/openssl' directory), the guard
names try to match the path specified in the include directives, with
all letters converted to upper case and '/' and '.' replaced by '_'. For the
private header files files, an extra 'OSSL_' is added as prefix.

Apart from public and internal header files, there is a third type called
local header files, which are located next to source files in the source
directory. Currently, they have different suffixes like
'*_lcl.h', '*_local.h', or '*_int.h'
This commit changes the different suffixes to '*_local.h' uniformly.

If application uses any of Windows-specific interfaces, make it
application developer's respondibility to include <windows.h>.
Rationale is that <windows.h> is quite "toxic" and is sensitive
to inclusion order (most notably in relation to <winsock2.h>).
It's only natural to give complete control to the application developer.

Some platforms claim to be POSIX but their getcontext() implementation
does not work. Therefore we update the ASYNC_is_capable() function to test
for this.

Implementation experience has shown that the original plan for async wait
fds was too simplistic. Originally the async logic created a pipe internally
and user/engine code could then get access to it via API calls. It is more
flexible if the engine is able to create its own fd and provide it to the
async code.
Another issue is that there can be a lot of churn in the fd value within
the context of (say) a single SSL connection leading to continually adding
and removing fds from (say) epoll. It is better if we can provide some
stability of the fd value across a whole SSL connection. This is
problematic because an engine has no concept of an SSL connection.
This commit refactors things to introduce an ASYNC_WAIT_CTX which acts as a
proxy for an SSL connection down at the engine layer.

In the async code for MacOS/X define _XOPEN_SOURCE (if not already
defined) as early as possible. We must do this before including
any header files, because on MacOS/X <stlib.h> includes <signal.h>
which includes <ucontext.h>. If we delay defining _XOPEN_SOURCE
and include <ucontext.h> after various system headers are included,
we are very likely to end up with the wrong (truncated) definition
of ucontext_t.
Also, better error handling and some code cleanup in POSIX fibre
construction and destruction. We make sure that async_fibre_makecontext()
always initializes the fibre to a state that can be freed.
For all implementations, check for error returns from
async_fibre_makecontext().

In theory the pthreads approach for Thread Local Storage should be more
portable.
This also changes some APIs in order to accommodate this change. In
particular ASYNC_init_pool is renamed ASYNC_init_thread and
ASYNC_free_pool is renamed ASYNC_cleanup_thread. Also introduced ASYNC_init
and ASYNC_cleanup.

A lot of the pool handling code was in the arch specific files, but was
actually boiler plate and the same across the implementations. This commit
moves as much code as possible out of the arch specific files.

We were using _pipe to create a pipe on windows. This uses the "int" type
for its file descriptor for compatibility. However most windows functions
expect to use a "HANDLE". Probably we could get away with just casting but
it seems more robust to use the proper type and main stream windows
functions.

Even with _XOPEN_SOURCE defined OS-X still displays warnings that
makecontext and friends are deprecated. This isn't a problem until you
try and build with --strict-warnings, and the build fails. This change
suppresses the warnings. We know they are deprecated but there is no
alternative!

async_fibre_makecontext was initialise the fibre first and then calling
getcontext(). It should be the other way around because the getcontext
call may overwrite some of the things we just initialised. This didn't
cause an issue on Linux and so the problem went unnoticed. On OS-X it
causes a crash.

_longjmp/_setjmp do not manipulate the signal mask whilst
longjmp/setjmp may do. Online sources suggest this could result
in a significant speed up in the context switching.
