This includes a general correction in the code (now using the X509V3_CTX_REPLACE flag)
and adding a prominent clarification in the documentation:
If multiple entries are processed for the same extension name,
later entries override earlier ones with the same name.
This is due to an RFC 5280 requirement - the intro of its section 4.2 says:
A certificate MUST NOT include more than one instance of a particular extension.

- In order to not add many X509_XXXX_with_libctx() functions the libctx and propq may be stored in the X509 object via a call to X509_new_with_libctx().
- Loading via PEM_read_bio_X509() or d2i_X509() should pass in a created cert using X509_new_with_libctx().
- Renamed some XXXX_ex() to XXX_with_libctx() for X509 API's.
- Removed the extra parameters in check_purpose..
- X509_digest() has been modified so that it expects a const EVP_MD object() and then internally it does the fetch when it needs to (via ASN1_item_digest_with_libctx()).
- Added API's that set the libctx when they load such as X509_STORE_new_with_libctx() so that the cert chains can be verified.

Update ParseC to handle this. Most of all, ParseC needed to be more
consistent. The handlers are "recursive", in so far that they are called
again and again until they terminate, which depends entirely on what the

CAkeyform may be set to PEM, DER or ENGINE, but the current options
are not using the proper optionformat 'E' (OPT_FMT_PDE) for this.
Set the valtype for CAkeyform to 'E' and use OPT_FMT_PDE when extracting
the option value.
This amends 0ab6fc79a9 ("Fix regression on x509 keyform argument") which
did the same thing for keyform and changed the manpage synopsis entries
for both keyform and CAkeyform but did not change the option section.
Hence, change the option section for both of them.

OpenSSL 1.1.0 has extended option checking, and rejects passing a PKCS#11
engine URL to "-signkey" option. The actual code is ready to take it.
Change the option parsing to allow an engine URL to be passed and modify
the manpage accordingly.

In OpenSSL pre 1.1.0, 'openssl x509 -keyform engine' was possible
and supported. In 1.1.0, type of keyform argument is OPT_FMT_PEMDER
which doesn't support engine. This changes type of keyform argument
to OPT_FMT_PDE which means PEM, DER or engine and updates the manpage
including keyform and CAkeyform.
This restores the pre 1.1.0 behavior.
This issue is very similar than https://github.com/openssl/openssl/issues/4366

Use `strrchr` to get a pointer to the last occurrence of `.` in the
path string, instead of the first one with `strchr`. This prevent the
path to be wrongly split if it contains several `.`, and not only the
one for the extension.

Everything in apps includes apps.h, because that one declares apps
internal library routines. However, progs.h doesn't declare library
routines, but rather the main commands and their options, and there's
no reason why the library modules should include it.
So, remove the inclusion of progs.h from apps.h and add that inclusion
in all command source files.

Standardized the -rand flag and added a new one:
-rand file...
Always reads the specified files
-writerand file
Always writes to the file on exit
For apps that use a config file, the RANDFILE config parameter reads
the file at startup (to seed the RNG) and write to it on exit if
the -writerand flag isn't used.
Ensured that every app that took -rand also took -writerand, and
made sure all of that agreed with all the documentation.

In apps/apps.c, one can set up an engine with setup_engine().
However, we freed the structural reference immediately, which means
that for engines that don't already have a structural reference
somewhere else (because it's a built in engine), we end up returning
an invalid reference.
Instead, the function release_engine() is added, and called at the end
of the routines that call setup_engine().

Add -DBIO_DEBUG to --strict-warnings.
Remove comments about outdated debugging ifdef guards.
Remove md_rand ifdef guarding an assert; it doesn't seem used.
Remove the conf guards in conf_api since we use OPENSSL_assert, not assert.
For pkcs12 stuff put OPENSSL_ in front of the macro name.
Merge TLS_DEBUG into SSL_DEBUG.
Various things just turned on/off asserts, mainly for checking non-NULL
arguments, which is now removed: camellia, bn_ctx, crypto/modes.
Remove some old debug code, that basically just printed things to stderr:
DEBUG_PRINT_UNKNOWN_CIPHERSUITES, DEBUG_ZLIB, OPENSSL_RI_DEBUG,
RL_DEBUG, RSA_DEBUG, SCRYPT_DEBUG.
Remove OPENSSL_SSL_DEBUG_BROKEN_PROTOCOL.

Not all architectures have a time_t defined the same way. To make
sure we get the same result, we need to cast &checkoffset to (intmax_t *)
and make sure that intmax_t is defined somehow.
To make really sure we don't pass a variable with the wrong size down
to opt_imax(), we use a temporary intmax_t.

Loading the config file after processing command line options can
cause problems, e.g. where an engine provides new ciphers/digests
these are not then recoginised on the command line. Move the
default config file loading to before the command line option
processing. Whilst we're doing this we might as well centralise
this instead of doing it individually for each application. Finally
if we do it before the OpenSSL_add_ssl_algorithms() call then
ciphersuites provided by an engine (e.g. GOST) can be available to
the apps.

The different apps had the liberty to decide whether they would open their
input and output files in binary mode or not, which could be confusing if
two different apps were handling the same type of file in different ways.
The solution is to centralise the decision of low level file organisation,
and that the apps would use a selection of formats to state the intent of
the file.

When generating a private key, try to make the output file be readable
only by the owner. Put it in CHANGES file since it might be noticeable.
Add "int private" flag to apps that write private keys, and check that it's
set whenever we do write a private key. Checked via assert so that this
bug (security-related) gets fixed. Thanks to Viktor for help in tracing
the code-paths where private keys are written.

Here are the "rules" for handling flags that depend on #ifdef:
- Do not ifdef the enum. Only ifdef the OPTIONS table. All ifdef'd
entries appear at the end; by convention "engine" is last. This
ensures that at run-time, the flag will never be recognized/allowed.
The next two bullets entries are for silencing compiler warnings:
- In the while/switch parsing statement, use #ifdef for the body to
disable it; leave the "case OPT_xxx:" and "break" statements outside
the ifdef/ifndef. See ciphers.c for example.
- If there are multiple options controlled by a single guard, OPT_FOO,
OPT_BAR, etc., put a an #ifdef around the set, and then do "#else"
and a series of case labels and a break. See OPENSSL_NO_AES in cms.c
for example.

The file name given to -CAserial might not exist yet. The
-CAcreateserial option decides if this is ok or not.
Previous to this change, -CAserial was a type '<' option, and in that
case, the existence of the file given as argument is tested quite
early, and is a failure if it doesn't. With the type 's' option, the
argument is just a string that the application can do whatever it
wants with.

Make setup_engine be a dummy if NO_ENGINE is enabled.
The option is not enabled if NO_ENGINE is enabled, so the one "wasted"
