c48ffbcca1 SSL: refactor all SSLfatal() calls
Since SSLfatal() doesn't take a function code any more, we drop that
argument everywhere.  Also, we convert all combinations of SSLfatal()
and ERR_add_data() to an SSLfatal_data() call.

3e5826061b Add helper functions for FreeBSD KTLS.
These are similar to the helpers added in 95badfeb60.  I've adjusted
the arguments passed to ktls_check_supported_cipher and
ktls_configure_crypto so that FreeBSD and Linux can both use the same
signature to avoid OS-specific #ifdef's in libssl.  This also required
moving the check on valid TLS versions into
ktls_check_supported_cipher for Linux.  This has largely removed
OS-specific code and OS-specific #ifdef's for KTLS outside of
<internal/ktls.h>.

d9c2fd51e2 The EVP_MAC functions have been renamed for consistency. The EVP_MAC_CTX_*
functions are now EVP_MAC functions, usually with ctx in their names.

Before 3.0 is released, the names are mutable and this prevents more
inconsistencies being introduced.

There are no functional or code changes.
Just the renaming and a little reformatting.

c8f6c28a93 Explicitly fetch ciphers and digests in libssl
We modify libssl to use explicitly fetched ciphers, digests and other
algorithms as required based on the configured library context and
property query string for the SSL_CTX that is being used.

8b6ffd4040 Params: change UTF8 construct calls to avoid explicit strlen(3) calls.
It is better, safer and smaller to let the library routine handle the
strlen(3) call.

706457b7bd Reorganize local header files
Apart from public and internal header files, there is a third type called
local header files, which are located next to source files in the source
directory. Currently, they have different suffixes like

  '*_lcl.h', '*_local.h', or '*_int.h'

This commit changes the different suffixes to '*_local.h' uniformly.

555cbb328e Collapse ssl3_state_st (s3) into ssl_st
With the removal of SSLv2, the s3 structure is always allocated, so
there is little point in having it be an allocated pointer. Collapse
the ssl3_state_st structure into ssl_st and fixup any references.

This should be faster than going through an indirection and due to
fewer allocations, but I'm not seeing any significant performance
improvement; it seems to be within the margin of error in timing.

7426cd343d Ensure that we write out alerts correctly after early_data
If we sent early_data and then received back an HRR, the enc_write_ctx
was stale resulting in errors if an alert needed to be sent.

Thanks to Quarkslab for reporting this.

In any case it makes little sense to encrypt alerts using the
client_early_traffic_secret, so we add special handling for alerts sent
after early_data. All such alerts are sent in plaintext.

43a0f2733a Fix some TLSv1.3 alert issues
Ensure that the certificate required alert actually gets sent (and doesn't
get translated into handshake failure in TLSv1.3).

Ensure that proper reason codes are given for the new TLSv1.3 alerts.

Remove an out of date macro for TLS13_AD_END_OF_EARLY_DATA. This is a left
over from an earlier TLSv1.3 draft that is no longer used.

4ff1a52666 Fix TLSv1.3 ticket nonces
All tickets on a connection need to have a unique nonce. When this was
originally implemented we only ever sent one ticket on the conneciton so
this didn't matter. We were just using the value 0. Now we can get multiple
tickets to we need to start doing the ticket nonce properly.

16cfc2c90d Don't use a ssl specific DRBG anymore
Since the public and private DRBG are per thread we don't need one
per ssl object anymore. It could also try to get entropy from a DRBG
that's really from an other thread because the SSL object moved to an
other thread.
