d2i_X509_bio(), d2i_X509_fp(), i2d_X509_bio(), and i2d_X509_fp()
were documented in OpenSSL 1.0.2. In a grand unification of the
documentation of (almost) all d2i and i2d functions, these were
dropped, most likely by mistake.
This simply adds them back.

Also includes CRMF (RFC 4211) and HTTP transfer (RFC 6712).

('=for comment' etc). This is abusive and confusing. Instead, we use
our own keyword.

Furthermore, for TYPE used as a placeholder for types and correponding
part of function names, we extrapolate that it's both a type and a
variable, and should therefore be bold (typical for types and function
names) and italic (typical for variables). POD processors don'e know
this, so we have to help them along. Therefore:
SPARSE_ARRAY_OF(TYPE) => B<SPARSE_ARRAY_OF>(B<I<TYPE>>)
ossl_sa_TYPE_num() => B<ossl_sa_I<TYPE>_num>()
TYPE => B<I<TYPE>>
There are some other less typical uses where one simply has to give
formatting some extra though.

They incorrectly said that i2d_ECDSA_SIG returns 0 on error. In fact it
returns a negative value on error.
We fix this by moving the i2d_ECDSA_SIG/d2i_ECDSA_SIG docs onto the same
page as all the other d2i/i2d docs.

The d2i docs state that if an error occurs then |*a| is not freed. This
is not correct. On error it is freed and set to NULL. We update the docs
to say this, and also discuss the fact that this behaviour was inconsistent
prior to OpenSSL 1.1.0.
