Including e_os.h with a path from a header file doesn't work well on
certain exotic platform. It simply fails to build.
Since we don't seem to be able to stop ourselves, the better move is
to move e_os.h to an include directory that's part of the inclusion
path given to the compiler.

The OPENSSL_memdup() is not always success, as the potential failure of
the allocation.
Then the '*pptr'could be NULL pointer but the ui_dup_method_data() will
still return 1.
In CRYPTO_dup_ex_data(), the 'storage[i]->dup_func' will not fail and
'ptr' will be used in CRYPTO_set_ex_data().
Also, if '*pptr' is NULL, I think it should also return 0 to tell the
caller that the duplication fails in order to prevernt using the NULL
pointer.
Therefore, it should be better to add the check and return 1 only if the
duplication succeed.

When `PEM_write_bio_PKCS8PrivateKey()` was passed an empty passphrase
string, `OPENSSL_memdup()` was incorrectly getting used for 0 bytes size
allocation, which resulted in malloc failures.

Some functions that lock things are void, so we just return early.
Also make ossl_namemap_empty return 0 on error. Updated the docs, and added
some code to ossl_namemap_stored() to handle the failure, and updated the
tests to allow for failure.

This fixes a strict aliasing issue in ui_dup_method_data.
The parameter type of CRYPTO_EX_dup's from_d parameter
is in fact void **, since it points to a pointer.
This function is rarely used, therefore fix the param type
although that may be considered an API breaking change.

Move some ASN1 struct typedefs to types.h
Update ParseC to handle this. Most of all, ParseC needed to be more
consistent. The handlers are "recursive", in so far that they are called
again and again until they terminate, which depends entirely on what the
"massager" returns. There's a comment at the beginning of ParseC that

Make the include guards consistent by renaming them systematically according
to the naming conventions below
For the public header files (in the 'include/openssl' directory), the guard
names try to match the path specified in the include directives, with
all letters converted to upper case and '/' and '.' replaced by '_'. For the
private header files files, an extra 'OSSL_' is added as prefix.

Apart from public and internal header files, there is a third type called
local header files, which are located next to source files in the source
directory. Currently, they have different suffixes like
'*_lcl.h', '*_local.h', or '*_int.h'
This commit changes the different suffixes to '*_local.h' uniformly.

The output C code was made to use ERR_func_error_string() to see if a
string table was already loaded or not. Since this function returns
NULL always, this check became useless.
Change it to use ERR_reason_error_string() instead, as there's no
reason to believe we will get rid of reason strings, ever.
To top it off, we rebuild all affected C sources.

This is a bit annoying, if for instance "openssl genrsa -aes128"
tries to read a 4+ character size password, but CTRL-C does no longer
work after a RETURN key, since the flag UI_FLAG_REDOABLE is set by
UI_set_result_ex, together with the error "You must type in 4 to 1023 characters".
Thus remove the REDOABLE flag to allow CTRL-C to work.
