Some functions that lock things are void, so we just return early.
Also make ossl_namemap_empty return 0 on error. Updated the docs, and added
some code to ossl_namemap_stored() to handle the failure, and updated the
tests to allow for failure.

Apart from public and internal header files, there is a third type called
local header files, which are located next to source files in the source
directory. Currently, they have different suffixes like
'*_lcl.h', '*_local.h', or '*_int.h'
This commit changes the different suffixes to '*_local.h' uniformly.

Make LHASH_OF use static inline functions.
Add new lh_get_down_load and lh_set_down_load functions and their
typesafe inline equivalents.
Make lh_error a function instead of a macro.

There are header files in crypto/ that are used by a number of crypto/
submodules. Move those to crypto/include/internal and adapt the
affected source code and Makefiles.
The header files that got moved are:

For a local variable:
TYPE *p;
Allocations like this are "risky":
p = OPENSSL_malloc(sizeof(TYPE));
if the type of p changes, and the malloc call isn't updated, you
could get memory corruption. Instead do this:
p = OPENSSL_malloc(sizeof(*p));
Also fixed a few memset() calls that I noticed while doing this.

changes are the fallout). As this could break source code that doesn't
directly include headers for interfaces it uses, changes to recursive
includes are covered by the OPENSSL_NO_DEPRECATED symbol. It's better to
define this when building and using openssl, and then adapt code where
necessary - this is how to stay current. However the mechanism exists for
the lethargic.
ENGINE_TABLE-based stuff - as described in crypto/engine/README.
Associated miscellaneous changes;
- the previous cipher/digest hooks that hardwired directly to EVP's
OBJ_NAME-based storage have been backed out. New cipher/digest support
has been constructed and will be committed shortly.
- each implementation defines its own ENGINE_load_<name> function now.
- the "openssl" ENGINE isn't needed or loaded any more.
- core (not algorithm or class specific) ENGINE code has been split into
multiple files to increase readability and decrease linker bloat.
- ENGINE_cpy() has been removed as it wasn't really a good idea in the
first place and now, because of registration issues, can't be
meaningfully defined any more.
- BN_MOD_EXP[_CRT] support is removed as per the README.
- a bug in enginetest.c has been fixed.
NB: This commit almost certainly breaks compilation until subsequent
changes are committed.
