BIO_get_ktls_send() and BIO_get_ktls_recv() are documented as
returning either 0 or 1. However, they were actually returning the
internal value of the associated BIO flag for the true case instead of
1.
Also trim redundant ternary operators.

Apart from public and internal header files, there is a third type called
local header files, which are located next to source files in the source
directory. Currently, they have different suffixes like
'*_lcl.h', '*_local.h', or '*_int.h'
This commit changes the different suffixes to '*_local.h' uniformly.

This commit adds comments to bio_method_st definitions where the
function pointers are defined as NULL. Most of the structs have comments
but some where missing and not all consitent.

I noticed that some of the BIO_METHOD structs are placing the name on
the same line as the type and some don't. This commit places the name
on a separate line for consistency (which looks like what the majority
do)

Because of the way bio_lcl.h is organised, we must not include
internal/cryptlib.h before it. As a matter of fact, bio_lcl.h
includes internal/cryptlib.h on its own.

affected source code and Makefiles.
The header files that got moved are:
crypto/cryptolib.h
crypto/md32_common.h
