The SSL3 prefix no longer seems appropriate. We choose TLS_RL_RECORD instead
of TLS_RECORD because that type already exists elsewhere.

The 1 in DTLS1 is confusing and is removed. We also tweak the structure
to always be able to track 64 packets regardless of whether we are on a
32 bit or 64 bit system.

Typedefs of CRYPTO malloc, realloc and free.
MEM_CHECK "modes" are used only as a CRYPTO_mem_ctrl() parameter
The CRYPTO_mem_ctrl is defined only if OPENSSL_NO_CRYPTO_MDEBUG is
defined, thus define the MEM_CHECK modes under the same condition.
Maybe the macros can be removed at all since:
1. CRYPTO_mem_ctrl() just returns -1 and ignores the parameter
2. CRYPTO_mem_ctr() is declared as DEPRECATED by 3.0

Trim trailing whitespace. It doesn't match OpenSSL coding standards,
AFAICT, and it can cause problems with git tooling.
Trailing whitespace remains in test data and external source.

Remove ERR_[gs]et_implementation as they were not undocumented and
useless (the data structure was opaque).
Halve the number of lock/unlock calls in almost all ERR_
functions by letting the caller of get_hash or int_thread_set
able to lock. Very useful when looping, such as adding errors,
or when getting the hash and immediately doing a lookup on it.

This is merges the old "rsalz-monolith" branch over to master. The biggest
change is that option parsing switch from cascasding 'else if strcmp("-foo")'
to a utility routine and somethin akin to getopt. Also, an error in the
command line no longer prints the full summary; use -help (or --help :)
for that. There have been many other changes and code-cleanup, see
bullet list below.
Special thanks to Matt for the long and detailed code review.
