Remove TLS heartbeat, disable DTLS heartbeat
To enable heartbeats for DTLS, configure with enable-heartbeats. Heartbeats for TLS have been completely removed. This addresses RT 3647 Reviewed-by: Richard Levitte <levitte@openssl.org>
