2770fc7ac6 outlib: add infrastructure for common section and symbol handling
Pretty much all the backends have to do the same gymnastics to handle
symbols and sections. In the future, this should be done by the
assembly core, but in order to prepare the ground and get the
performance benefits as soon as possible, implement a library of
functions which can manage symbols and sections and their respective
hashes, trees, and tables.

This infrastructure is not yet used by any backend, that porting work
has to come next.

e1f985c167 Reorganize the source code into subdirectories
Make the source code easier to understand and keep track of by
organizing it into subdirectories depending on the function.

55ae12052c Add support for one-byte relocations
Add OUT_REL1ADR (one-byte relative address) and support for
OUT_ADDRESs with size == 1.  Add support for it in
outbin and outdbg.  *It still needs to be added to other backends*,
both the OUT_REL*ADR and OUT_ADDRESS codepaths need to be handled.

Add new copyright headers to the new output modules.  As far as I
know, the only module which we still don't have a green light to
release under 2-BSD is outmacho.

31b707bef0 Move backend-specific code to output/; break out null debug stuff
Move backend-specific code into the output/ directory, and make the
null debugging backend a separate file (it certainly isn't needed for
ndisasm...)
