* stmt.c (cost_table): Remove.
        (COST_TABLE, cost_table_initialized): New.
        (estimate_case_costs): Use the later instead of the former.
        (balance_case_nodes): Likewise.

debuggers, etc., plus their support routines, definitions, and documentation.

instructions.
