start of a line, treat it as a comment character if it is one.
For a CPP line comment use pseudo-op .appline rather than .line.

logical_input_line to -1 rather than 0.
(bump_line_counters): Increment logical_input_line.
(new_logical_line): If line_number is -2, decrement
